I live here in 520 east Carson st space 40, carson CA. I am a gsis pensioner and I just had gsis e card plus replaced last September 2014. When I went home in the Philippines and have been using it here in withdrawing my pension here in California since then. Last Sept 2015 I was told on the ATM (bank of America) that my card is expired so I can’t withdraw my pension when it is not damaged or it is expired. ![]() I request you to re activate my card as my pension is important to me and my family. My unactivated UMID Card was lost last september, 2010 and appropriate affidavit of loss was already submitted to the GSIS through our Department’s authorized representative. Furtheremore, I already coordinated with the Union bank for the blocking of my activated e- card. Nonetheless during submission of my affidavit of loss, the union bank told me that they can not yet make a replacement of my lost UMID card since they still have to wait for the release of guidelines thereon. ![]() In view of the foregoing, please provide me accurate information on the following matters: 1. Sometime last 2009 i came to central office to inquire why i cannot access my account, after verification, i was told that it was indeed “LOCKED’ for the reasons even the one attending me cannot explain why. ( for Christ sake how could this be???? He toldme to request our field office in Puerto Princesa to “UNLOCK” my records, i make a request for that reason, that was last year. UMID Officer In-Charge Sir/Madam: My officemates and I applied for a UMID card last June 19, 2010, and until now four of us have not received our cards yet. This is the first time I applied for said card, along with my other officemates who were appointed as permanent employees two years ago. I wonder why we were not issued our card while my other officemates were issued theirs. We were on the same standing as newly-appointed employees and applied on teh same day. I have kept on calling the GSIS office here in Puerto Princesa City and they explained that they are still waiting for a signal from SSS on the issuance of CRN or the reference number. I was issued a temporary one and lost it, I inquired at the Gsis office and informed me that they won’t be replacing my temporary ecard anymore and that I should wait for my UMID card. Nobody from GSIS cannot give me a straight answer. I am having a hard time to go to your branch to file a loan and always bring a new affidavit of loss every time. Please help me its not funny paying for the fare and fee at the notary public all the time.
